The customer will submit his request for bulk telephone number Lookups as follows:

1. Submit a disk formatted in DOS file format, one 10-digit U.S. or Canadian telephone number per line.

2. Numerically sort phone numbers by area code (NPA), then by central office (NXX), then by station (XXXX). (Ordinary numerial order). If not sorted, Hattrick Publishing Group will sort before lookup, and the output file will contain phone numbers in numerical order, plus names and addresses.

The customer will receive a disk of Bulk Telephone Number Lookups in the following format:

1. DOS file format, five lines per input telephone number. If the number lookup yields two listings the output will contain 10 lines.

2. First line contains the input telephone number. If the lookup yields a residence (business), we append "[R]" ("[B]") to the number, the next three lines contain the listing name, listing street address, and city-state-zip code, respectively. If no match found, the three lines following the phone number contain the words NO, MATCH, FOUND, respectively. The fourth line after the phone number is always blank. If the number lookup yields two listings the output is two five-line number-name-address sequences, and "[D]" (double) is appended to both phone number lines.

The cost of this service is 4 cents per match, with a $29.00 fee for shipping and handling. The minimum order is $150.00, payable in advance by check made out to "HATTRICK PUBLISHING GROUP." The minimum order fee must be received before a job will be started. The balance must be paid before the search results will be delivered.

Search results will be mailed by "Priority Mail" unless delivery by FED-EX is requested. There is an additional $15.00 charge for FED-EX next business day delivery. There will be no charge for matches not found. Customers may expect orders to be completed within 10 business days.

The Customer, by signature below, agrees to these terms and conditions for the use of "Match-A-Name."
