The customer will submit his request for a fax on demand program as follows:

1. Submit the document(s) that will be stored on our fax server and available through our Fax On Demand system by fax to 1-800-533-2FAX (2329) or by email as PDF file to lauren@telecompute.com
   
   
2. Submit a statement of how that fax documents are to be accessed with a flow chart if possible, i.e. press 1 for Document 1, press 2 for Document 2, etc.
   
   
3. Telecompute will provide a toll free fax number to the customer for access to the Fax On Demand system that the customer may publicize.
   
   
4. In the alternative, all materials may be mailed or sent by express delivery service to:
